Attention New Patients:

an intake form is to be completed electronically 24 hours prior to your first scheduled visit. There will be a link via email once your appointment is booked. The intake is to be filled out with an electronic signature. If this is not completed in this timeline, the RMT will email/call/text. If there is no communication the appointment will face cancellation.

Direct Billing is Available for most insurance plans: please fill out information prior to arrive when you get sent your welcome email
